Federal COVID-19 Vaccine Requirement

President Biden’s Executive Order on vaccines requires all employees of federal contractors to receive COVID-19 vaccinations or request and have approved a reasonable accommodation based on a medical need or a sincerely held religious belief. Because MCCCD has covered federal contracts, this Executive Order applies to all employees of our System including faculty, staff, student workers, full/ part-time employees, temporary employees and volunteers.

Per the mandate, this COVID-19 vaccination requirement applies to all employees regardless of remote or virtual status.

All employees must be fully vaccinated for COVID-19 by January 7, 2022, or have applied for religious or medical accommodation.

To be in compliance individuals must complete the following:

  • Federal COVID-19 Vaccine Mandate Acknowledgement (Available in the Employee Learn Center) - Required by All
  • Federal COVID-19 Mandate Vaccine Documentation or Intent to Request an Accommodation Form (Available through a unique email link) - Required by All
  • If requesting an accommodation, submit either the Medical/Disability Accommodation Request Form or Religious Accommodation Request Form (Available Below) - Required of all persons who do not submit Fully Vaccinated Documentation

Submit Your Federal Vaccine Mandate Documentation or Intention to Request an Accommodation


Employees can submit their vaccine documentation through the secure, Federal COVID-19 Vaccine Documentation form. Employees requesting an accommodation must also submit their intention to request an accommodation through the vaccine documentation form. 

Employees must submit one of the following:

  • A copy of the record of immunization from a health care provider or pharmacy
  • A copy of the COVID-19 Vaccination Record Card
  • A copy of medical records documenting the vaccination
  • A copy of immunization records from a public health or state immunization information system
  • A copy of any other official documentation verifying vaccination with information on the vaccine name, date(s) of administration, and the name of health care professional or clinic site administering the vaccine

Digital copies such as photographs, scans, or PDFs are eligible. Vaccine document needs to be in English or have an English translation submitted along with a copy of the original document.

Vaccine Documentation or Intent to Request an Accommodation can now be submitted through the unique email link that will be sent directly to you. Emails will be sent every Monday until you complete the form.

Vaccine Mandate or Accommodation Due Date

Individuals need to be Fully Vaccinated (meaning two weeks have passed after the single or second dose) by this date OR Request an accommodation by 1/7/22.

Vaccine Information

The following vaccines are eligible for the Federal COVID-19 Vaccine Mandate:

  • Moderna 
  • Pfizer
  • Johnson & Johnson/Janssen
  • AstraZeneca/Oxford
  • Novavax

Request an Accommodation


Federal guidelines allow for accommodations to the vaccine requirement based on medical /disability needs and sincerely held religious beliefs. If you are unable to be fully vaccinated due to one of the reasons, you must submit an accommodation request. Requests for an accommodation must be submitted by January 7, 2022.

General philosophical or moral objections to receiving a vaccine or being fully vaccinated shall not suffice as the basis for an approved accommodation.

How to Submit an Accommodation Request:

  1. Select the appropriate button above to access the accommodation form. Both the Medical/Disability and Religious accommodation requests will be submitted through dynamic forms.
  2. Fully complete the form and click the "submit" button. 
  3. The form will be routed directly to the Human Resources Solution Center (HRSC) for review. All requests will be reviewed centrally. 
  4. You will receive an acknowledgement email once you have submitted the request form. If you did not receive the acknowledgement please contact HRSC to ensure your form was submitted successfully. 
  5. HRSC will reach ou to you to begin the interactive process under the Americans with Disabilities Act (ADA)/ND-5 Reasonable Accommodations Process for Medical/Disability requests. 

Once the process is complete, you will receive a final notification from the Human Resources Solution Center notifying you if the request was approved. If the request was approved, the notification will include the designated accommodation process and next steps. 


New Employees


Vaccine Timeline for New Employees

  • New employees hired prior to January 7, will be required to provide documentation of full immunization within 30 days of their start date if their start date*
  • New employees hired after January 7, will be required to provide documentation of full immunization prior to their start date as a condition of employment*

*Unless otherwise exempted from this Policy by an approved accommodation pursuant to the interactive process.

  • Initial notice on the Careers website
  • Included on each job posting
  • Written and verbal notification as part of the interview process
  • Verbal notification in the job offer call
  • Written notification in the job offer letter
